Building an English-speaking patient base beyond an established Chinese-speaking market

An established plastic surgery clinic wanted to develop an English-speaking patient base. growly connected its strengths with procedure demand, then built high-intent content, a new website and relevant external visibility. By month three after launch, AI-attributed inquiries from the US alone had risen from zero to 74 per month.

The priority was a lasting search presence in a new market. The clinic already had a successful business; the starting point was helping English-speaking prospects discover and evaluate its established strengths.

01 / The growth objective

A successful clinic looked for its next patient base

The clinic already operated a successful business centered on Chinese-speaking patients. Its next objective was an English-speaking patient base. Immediate cash flow was not the pressing need. It wanted to build a longer-term presence through which new customers could discover and evaluate the clinic in Google and AI search.

An established clinical offering remains valuable in a new market, but the way customers discover and understand it does not transfer automatically. The work needed to begin with the procedures English-speaking prospects sought and the conditions they compared. The question was which demand to pursue first and what information would address it.

02 / The website challenge

The procedure list needed to support a choice

The existing website largely listed procedures. Schema and hreflang were underdeveloped, and the site lacked connected coverage of related questions and supporting information. Creating a search and AI discovery presence for English-speaking prospects required work on both the content and its technical foundation.

Finding a procedure name does not complete a patient’s choice of clinic. The explanation needs to show the clinic’s relevant strengths, why it merits consideration and what else the prospect should investigate. Translating a list would create English pages without necessarily providing enough information to evaluate the clinic.

growly’s direction was to explain the clinic’s strengths on core pages, with FAQs and blog content addressing related questions. Technical elements supported how those explanations connected across language versions. The information needed to make the clinic’s expertise understandable mattered more than adding terminology or markup alone.

03 / Market and patient demand

Narrowing the investment to demand the clinic could serve well

We began by examining the clinic’s positioning and strengths alongside English-speaking prospects’ procedure demand. We reviewed Google search volumes and trends, questions and comparisons on Reddit, and interest expressed on Instagram. Search wording alone would not adequately explain why prospects were comparing particular procedures and clinics.

The research included economic motivations associated with procedures. The reasons to consider treatment abroad needed to align with the value the clinic could offer. A large search audience and an attractive investment for this clinic were separate judgments. The initial focus also considered the fit between prospective patients and the clinic’s brand.

Each source served a different purpose. Search data indicated interest and trends; questions and comparisons helped explain what prospective patients wanted to understand. We used them to decide which procedures and search intentions deserved content resources first, rather than combining them into a definitive market-size estimate.

04 / Search intent and content

Prioritizing searches with clear patient intent

For a new domain, competing first on broad category terms can require substantial time and resources. This project prioritized niche, high-intent long-tail keywords aligned with the clinic’s strengths. Search volume was one input. Equally important was whether the clinic could provide a relevant answer to the person making that search.

That choice shaped the website from the outset. Core pages addressing the target searches were planned alongside FAQs and blog content for related questions during evaluation. Each page had a role within a broader topic, with internal connections helping visitors continue their research and find a relevant route to inquiry.

The selected clinical topics extended beyond a single core page. Supporting content addressed related questions and connected back to the central explanation. Internal link mapping organized the route through that information; external link mapping considered connections beyond the website. Each piece needed a defined question to answer within the wider topic.

05 / The website build

A three-week website build with search and future advertising in mind

The clinic’s preference for a lasting Google and AI search presence informed the order of work. growly recommended establishing the on-site foundation first. Positioning, copy, page architecture and links were addressed within the website build, which took three weeks.

The site also had to support future performance marketing. We considered how the clinic’s strengths would be communicated in ads and carried through to the landing page. Whether a visitor arrived through search or a future campaign, the page needed to answer the question that brought them there and provide relevant reasons to consider the clinic.

The website was custom-built to implement the required structure. Core procedure pages, supporting questions and the internal and external link plan were connected within the build. Three weeks was the website production period, separate from the time needed for search results to emerge. Launching the site and developing its discovery presence were successive parts of the work.

Connecting the content plan with its technical foundation

We assigned a role to each page around the selected procedures, then implemented schema, hreflang and the internal link plan in the new website. These elements addressed the clinic and procedure information, relationships between language versions, and routes through related content.

Including them in the same build kept the content plan and technical implementation aligned. Deciding which procedure belonged on a core page and which questions needed supporting content gave the language relationships and internal links a structure to follow. Hreflang identified corresponding language and regional versions; internal links provided routes between related topics.1

Content and external visibility work continued on that foundation. Subsequent ranking and inquiry changes are assessed across the project as a whole; the contribution of an individual technical element was not isolated.

07 / AI discovery

Planning for AI discovery from the beginning

AEO was part of the original website plan. We defined the questions customers would ask about the selected procedures, the core explanation and the supporting information while planning the site.

Discovery through search and AI is useful only if customers can find a clear answer on the clinic’s own pages. Google’s AI Overviews and AI Mode do not require a special schema. For this project, the priority was a clear answer to each question, supporting information and routes to the relevant content.2

The on-site work established a place to answer prospective patients’ questions. The next task was discovery: introducing the new domain to relevant audiences and connecting it with information beyond the website.

08 / External channels and links

Selecting external places where prospects could discover the clinic

The website launched on a new domain. The clinic’s established business did not remove the need to create discovery routes for its new address. growly used Reddit PR as an external touchpoint where prospects asked questions and shared information. The work sought opportunities for the clinic’s information to enter that evaluation process.

External specialists supported backlink acquisition. Selection considered fit with the clinic’s brand and expected return relative to cost. A link count alone could not explain the purpose of the work. Keeping external connections relevant to the procedures and patients prioritized on the website maintained a consistent direction.

The sequence was to establish the core procedure pages and supporting content, then broaden opportunities for discovery. The website, related content and external visibility supported the same patient-acquisition objective. The work was not an experiment isolating the effect of external activity, so subsequent inquiry changes cannot be assigned to a particular post or link.

09 / Project outcomes

The new domain gained search visibility and monthly inquiries

The website took three weeks to build; the search results developed after launch. From approximately two and a half months after publication, all three selected niche, high-intent long-tail keywords ranked within Google’s top five. This is a result for the specific searches targeted in the engagement.

By the third month after launch, AI-attributed inquiries from the US alone reached 74 per month, up from zero before the project. Monthly Google search inquiries increased from 13 to 87, approximately 6.7 times the baseline. Alongside growth in search inquiries, the records also began to show inquiries classified as AI-attributed.

Average spend in the project’s anglophone patient cohort was 38% higher than in the comparison cohort of Chinese-speaking and Southeast Asian patients. Alongside inquiry volume, this showed the commercial characteristics of the patients served. The clinic had a basis for assessing the value of its expanded customer base.

These are observations before and after the project. Content, website development and external visibility were delivered together, so the figures do not isolate the effect of any single activity. Inquiries are separate from bookings, completed visits and payments. Search visibility and inquiries reflected progress towards the English-speaking acquisition objective; average spend added a view of the customer groups’ transaction characteristics.

Making an established strength accessible to a new audience

The clinic already had an established practice and a successful business. growly’s role was to make its strengths discoverable and understandable to a new audience. Research identified the demand to prioritize, the website addressed that demand, and search and external channels expanded the opportunities for discovery.

Developing an English-speaking patient base required more than translating a website. What patients searched for, the evidence they needed and the fit with the clinic’s goals informed the work throughout. That shared basis connected content, development, SEO, AEO and external visibility within the engagement.
